As with any insurance, you need to contact them to verify that your plan network includes services at our clinic. Inclusion of your company on this list does not guarantee that services will be covered.
For HMO plans, you will need to contact your Family Care Physician for a referral prior to beginning physical therapy. If you have any further questions regarding insurance coverage, please feel free to call our main office in Salem and ask to speak with the Billing Manager. She will be happy to assist you with your needs.
